## 15 February 1898 — USS Maine Explosion

The battleship USS Maine exploded and sank in Havana harbor, killing 266 sailors, and American newspapers immediately blamed a Spanish mine, helping push the United States into the Spanish-American War. Later technical investigations, including a 1976 U.S. Navy study, concluded the explosion most likely resulted from an internal coal-bunker fire igniting the ship's magazines, not external sabotage. Claims that the U.S. government itself staged the explosion as a pretext for war are a persistent but unproven conspiracy theory, not supported by the physical evidence.

## c. 1 January 1909 — Yevno Azef Exposed as Double Agent

Yevno Azef simultaneously led the Socialist Revolutionary Party's terrorist Combat Organization, which carried out assassinations of senior Russian officials, while secretly working as a paid informant for the Okhrana, the Tsarist secret police. Investigator Vladimir Burtsev exposed his dual role in early 1909, revealing that Azef had both organized real assassinations and betrayed fellow revolutionaries to the police, sometimes doing both to the same plot. His case remains a defining example of a state agent provocateur operating at the highest level of a revolutionary organization.

## c. 1 May 1917 — Roman Malinovsky Exposed as Okhrana Agent

Roman Malinovsky rose to become a leading Bolshevik Duma deputy and a close associate of Lenin while secretly serving as a paid Okhrana informant, reporting on party activities in exchange for money. After the February Revolution, opened Okhrana archives confirmed his double role, and he was later tried and executed by the Bolsheviks in 1918. His case is a documented instance of a state informant occupying a senior position inside the organization he was reporting on.

## c. 1 January 1921 — Operation Trust

The Soviet secret police (Cheka, later OGPU) created a fake anti-Bolshevik underground organization called the Monarchist Union of Central Russia, or 'the Trust,' to draw out and identify genuine White emigre opposition networks in Europe. Over several years the operation lured prominent anti-Soviet operatives, including British agent Sidney Reilly, back into Soviet territory, where they were captured. It is one of the most extensively documented state provocateur operations of the twentieth century.

## 25 September 1925 — Sidney Reilly Captured via the Trust

British intelligence agent Sidney Reilly, lured by the fake Trust organization run by Soviet secret police, crossed into the USSR believing he was meeting genuine anti-Bolshevik conspirators. He was arrested almost immediately and executed by the OGPU weeks later. His capture is documented as the culmination of the Operation Trust deception.

## 18 September 1931 — Mukden Incident

Japanese army officers secretly detonated a small charge on the South Manchuria Railway near Mukden and blamed it on Chinese saboteurs. The Kwantung Army used the staged blast as the pretext to invade and occupy all of Manchuria within months. Japanese investigations and postwar tribunals confirmed the bombing was carried out by Japanese officers themselves.

## 27 February 1933 — Reichstag Fire

The German parliament building was set ablaze weeks after Hitler became chancellor, and the Nazi government blamed a lone Dutch communist, Marinus van der Lubbe, who was tried and executed. The fire was used to suspend civil liberties and consolidate Nazi power. Whether the Nazis themselves secretly set the fire as a false-flag pretext, or opportunistically exploited a fire genuinely set by van der Lubbe acting alone, has been debated by historians for decades and remains formally unresolved; this event should be treated as contested, not settled.

## 7 July 1937 — Marco Polo Bridge Incident

A clash between Japanese and Chinese troops near the Marco Polo Bridge escalated into full-scale war between Japan and China. Unlike the Mukden Incident, most historians regard this clash as beginning from a genuine, unplanned skirmish, possibly triggered by a misfired shot or missing soldier, that was then deliberately escalated by Japanese commanders seeking pretext for wider war. Claims that it was a fully staged provocation from the outset, comparable to Mukden, are contested rather than established fact.

## 31 August 1939 — Gleiwitz Incident

SS operatives dressed as Polish soldiers staged an attack on a German radio station at Gleiwitz, leaving behind a murdered concentration camp prisoner dressed in a Polish uniform as fabricated evidence. Hitler cited the fake attack, along with a wider set of staged border incidents under Operation Himmler, as justification for invading Poland the next morning. The operation was confirmed by postwar testimony from SS officer Alfred Naujocks, who led it.

## 26 November 1939 — Mainila Shelling Incident

Soviet artillery shelled the Soviet village of Mainila and the government publicly blamed Finland for the attack, despite Finnish forces having withdrawn their artillery out of range beforehand. The Soviet Union used the incident to renounce its non-aggression pact with Finland and launch the Winter War four days later. Soviet officials and archival records later confirmed the shelling was carried out by the Red Army itself.

## 8 February 1950 — Stasi Informant Network Established

East Germany's Ministry for State Security, the Stasi, was founded and over the following decades built one of the largest civilian informant networks in history, recruiting hundreds of thousands of 'unofficial collaborators' (Inoffizielle Mitarbeiter) to surveil and report on friends, colleagues, and family. Post-reunification access to Stasi archives documented the scale of the program in detail. It stands as one of the most thoroughly documented state informant systems of the Cold War.

## 2 July 1954 — Lavon Affair

An Israeli military intelligence cell recruited local Egyptian Jews to plant bombs at Egyptian, American, and British civilian targets, including a post office and cinemas, intending for the attacks to be blamed on the Muslim Brotherhood or Egyptian nationalists and derail British withdrawal plans. The network was caught after a bomb ignited prematurely, and several agents were arrested, tried, and two executed. Israel officially acknowledged its role decades later, and the affair is formally recognized as a false-flag operation.

## 28 August 1956 — COINTELPRO Launched

The FBI formally launched COINTELPRO, a covert program using informants, infiltrators, and provocateurs to disrupt, discredit, and divide political organizations the Bureau viewed as threats, including civil rights groups, the Communist Party, and later the Black Panthers and antiwar movement. Tactics documented in later congressional investigations included planting agents who encouraged illegal acts and spreading forged letters to sow internal distrust. The program ran until 1971.

## c. 1 November 1956 — Operation Gladio Stay-Behind Network Established

The CIA and Italian military intelligence formalized a clandestine 'stay-behind' network, codenamed Gladio, intended to organize resistance in the event of a Soviet invasion, with parallel networks established across NATO countries in Western Europe. The network was armed, funded, and trained in secret, coordinated loosely through NATO's Clandestine Planning Committee. Its existence remained officially secret for over three decades.

## 13 March 1962 — Operation Northwoods Proposal

The U.S. Joint Chiefs of Staff drafted and formally proposed a set of false-flag operations, including staged terrorist attacks and a fabricated Cuban attack on a U.S. plane, meant to be blamed on Fidel Castro's government to justify a military invasion of Cuba. Defense Secretary Robert McNamara rejected the plan and it was never carried out. The proposal is documented through declassified Pentagon papers released in the late 1990s, not through any executed attack.

## 22 November 1963 — JFK Assassination 'Deep State' False-Flag Narratives

President John F. Kennedy was assassinated in Dallas, and the Warren Commission concluded Lee Harvey Oswald acted alone, a finding disputed by many Americans ever since. A wide range of narratives frame the assassination as a false-flag-style operation orchestrated by the CIA, military-industrial interests, or other state actors to redirect U.S. policy. No such narrative has been substantiated by declassified evidence, and this remains a contested and largely unproven set of conspiracy theories rather than an established fact.

## 2 August 1964 — Gulf of Tonkin Incident

A real naval skirmish occurred on August 2, 1964, between the USS Maddox and North Vietnamese torpedo boats. A second attack reported on August 4 was used by the Johnson administration to push the Gulf of Tonkin Resolution through Congress, escalating U.S. involvement in Vietnam, but that second attack did not happen as described. Declassified NSA history published in 2005 confirmed that officials knew the evidence for the second attack was unreliable when they presented it to Congress as fact.

## 25 March 1965 — Gary Rowe, FBI Informant, and the Murder of Viola Liuzzo

Gary Thomas Rowe was an FBI informant inside the Ku Klux Klan who was present in the car during the murder of civil rights volunteer Viola Liuzzo, though accounts of his exact role in the shooting remained disputed. Congressional investigations later documented that the FBI knew Rowe had participated in Klan violence, including beatings of Freedom Riders, while he remained on the Bureau's payroll. His case became a central example cited in the 1975 Church Committee's findings on FBI informant abuses.

## 4 December 1969 — William O'Neal, FBI Informant, and the Killing of Fred Hampton

William O'Neal was an FBI informant who infiltrated the Illinois Black Panther Party and provided the Bureau with a detailed floor plan of chairman Fred Hampton's apartment shortly before Chicago police, coordinating with the FBI, raided it and killed Hampton in his bed. Subsequent lawsuits and a 1976 congressional investigation confirmed O'Neal's role and FBI foreknowledge of the raid. The case is one of the most documented instances of a COINTELPRO informant directly enabling lethal state action.

## 12 December 1969 — Piazza Fontana Bombing

A bomb killed 17 people at a bank in Piazza Fontana in Milan, the opening act of Italy's 'strategy of tension,' a period of bombings and violence during the Cold War. Italian judicial investigations spanning decades eventually convicted members of the neofascist group Ordine Nuovo, and courts found evidence of complicity by figures within Italian military intelligence, though the full extent of any Gladio network involvement remains contested and was never conclusively proven in court. The case is documented through multiple Italian trials, even where some causal links remain disputed by historians.

## c. 1 May 1970 — Tommy the Traveler Provocateur Exposed

An FBI informant known on campus as 'Tommy the Traveler' spent months urging students at Hobart and William Smith Colleges to firebomb an ROTC building and other targets, supplying dynamite himself before the plot was exposed as an FBI-directed provocation. Local reporting and later congressional review confirmed he had manufactured the plot rather than merely observing an existing one. The case became a widely cited example of a provocateur creating the crime he was ostensibly investigating.

## 8 March 1971 — Media, Pennsylvania FBI Burglary Exposes COINTELPRO

A group of antiwar activists broke into an FBI field office in Media, Pennsylvania, and stole thousands of internal documents, which they mailed to journalists. The files provided the first public documentary evidence of COINTELPRO's existence and its use of informants and provocateurs against domestic political groups, forcing the FBI to formally end the program later that year. The burglars' identities were not revealed until 2014.

## 2 August 1980 — Bologna Railway Station Massacre

A bomb planted at Bologna's central railway station killed 85 people, the deadliest attack of Italy's 'strategy of tension' period. Italian courts convicted several neofascist militants of the bombing, and later trials found members of Italian military intelligence guilty of obstructing the investigation, though claims that the Gladio stay-behind network directly orchestrated the attack remain contested among historians and were not established as proven fact in the convictions. Italian parliamentary inquiries into the broader strategy-of-tension period continued for decades afterward.

## 10 October 1990 — Nayirah Incubator Testimony

A 15-year-old girl identified only as 'Nayirah' testified before Congress that Iraqi soldiers had removed babies from incubators in Kuwaiti hospitals and left them to die, testimony that was widely cited by U.S. officials building support for the Gulf War. It later emerged she was the daughter of Kuwait's ambassador to the U.S. and had been coached by the PR firm Hill & Knowlton as part of a campaign funded by the Kuwaiti government, with no independent verification of her account. The episode is a well-documented case of fabricated testimony used to manufacture public support for war.

## 24 October 1990 — Andreotti Confirms Gladio to Italian Parliament

Italian Prime Minister Giulio Andreotti publicly confirmed to Parliament the existence of Gladio, the secret NATO-linked stay-behind network that had operated in Italy since the 1950s. The disclosure triggered parliamentary investigations and revelations of parallel stay-behind networks in Belgium, France, Switzerland, and other NATO countries. Andreotti's confirmation remains the primary documented acknowledgment that anchors the wider Gladio story.

## 19 April 1995 — Oklahoma City Bombing Government-Complicity Claims

Timothy McVeigh bombed the Alfred P. Murrah Federal Building, killing 168 people, and was convicted and executed for the attack, with co-conspirator Terry Nichols also convicted. Persistent conspiracy claims allege the government had advance knowledge of the bombing or that additional, uncaught conspirators including federal informants were involved. These claims remain unproven and are treated as contested conspiracy theories rather than documented fact, distinct from McVeigh and Nichols's established, court-proven guilt.

## c. 11 September 2001 — 9/11 'Inside Job' False-Flag Claims

In the aftermath of the September 11 attacks, conspiracy theories emerged claiming the U.S. government secretly orchestrated or knowingly allowed the attacks as a pretext for war and domestic security expansion. These claims have been extensively investigated and rejected by the 9/11 Commission, independent engineering studies including NIST's structural analysis, and mainstream historians and journalists across the political spectrum. This is a widely circulated but unproven and largely debunked conspiracy theory, not a documented false-flag case.

## c. 1 January 2011 — Mark Kennedy Undercover Officer Exposed

British police officer Mark Kennedy spent seven years undercover inside environmental and anarchist protest groups under a false identity, forming long-term intimate relationships with activists and taking part in direct action, before activists uncovered his real identity in late 2010. Public exposure in January 2011 revealed his role had gone beyond intelligence-gathering into participation in and encouragement of protest activity, prompting several collapsed prosecutions and a formal review. His case triggered the wider UK 'spycops' scandal.

## c. 1 January 2011 — Undercover Officers' Deceptive Relationships Scandal

Following Mark Kennedy's exposure, further reporting revealed that multiple undercover UK police officers, including Bob Lambert, had formed long-term sexual relationships with activists under false identities, in some cases fathering children who did not know their father's real identity. Official acknowledgment and settlements followed, along with a Metropolitan Police apology in 2015. This documented pattern became a core focus of the subsequent public inquiry.

## 14 December 2012 — Sandy Hook 'False Flag' Claims

After a gunman killed 20 children and 6 adults at Sandy Hook Elementary School, media figure Alex Jones and others promoted the claim that the massacre was a staged 'false flag' event using crisis actors to build support for gun control. These claims were categorically false, caused severe harassment of victims' families, and resulted in defamation judgments exceeding a billion dollars against Jones in 2022. This is a clearly debunked conspiracy theory, not a contested or ambiguous case.

## 15 April 2013 — Boston Marathon Bombing 'Crisis Actor' Claims

Bombs planted by the Tsarnaev brothers at the Boston Marathon killed three people and injured hundreds, in an attack with an extensive public investigative and forensic record leading to one brother's death and the other's conviction. Online conspiracy theories nonetheless claimed the bombing was staged using government-directed crisis actors, similar to claims made about other mass-casualty attacks. These claims are unsupported by evidence and are treated as a debunked conspiracy theory.

## 6 July 2015 — UK Undercover Policing Inquiry Established

British Home Secretary Theresa May announced a statutory public inquiry into undercover policing after the Mark Kennedy and 'spycops' relationship scandals, examining decades of covert police infiltration of political groups dating back to 1968. The inquiry, chaired by a senior judge, has continued to hear evidence into the 2020s and has confirmed dozens of documented cases of officers using deception, false identities, and in some cases provocation while undercover. It remains the most extensive official documentation of state infiltration tactics in modern British history.

## 4 May 2020 — COVID-19 'Plandemic' False-Flag Claims

A viral video titled 'Plandemic' and related online claims alleged that the COVID-19 pandemic was deliberately engineered or exploited by global elites as a staged crisis to expand government control and profit pharmaceutical companies. Public health agencies, virologists, and fact-checking organizations found no credible evidence supporting a coordinated 'plandemic,' though legitimate, separate scientific debate continued over the virus's origins. The 'plandemic' framing itself is a widely circulated but unsubstantiated conspiracy claim, distinct from the genuinely unresolved lab-leak versus natural-origin question.
